At Logan Pediatrics, we believe everyone deserves access to high-quality, affordable health care. Our Sliding Fee Discount Program helps reduce the cost of medical services for individuals and families based on income and household size. This program is available at all Logan Pediatrics locations and applies to all services.

The Sliding Fee Discount Program is designed to help patients with limited income by offering significant discounts on health care costs. Patients at or below 200% of the Federal Poverty Level (FPL) may qualify for reduced rates, even if you already have insurance. 1. Download the Sliding Fee Application here or request a paper application from any Logan Pediatrics location.

2. Include proof of income (as outlined in the Sliding Fee Policy). If you have no income, submit a Self-Declaration Letter stating you currently receive no income for your household.

3. Submit your completed application and documents in person or via mail to:

4. After your application has been submitted, you will receive a decision letter in the mail within 30 days. If approved for the program, discounts will be valid for one year. Participants must re-apply to continue receiving discounts for the following year.
