- 6 Months Dtap #3, Hib #3, IPV #3, Hep B #3, PCV #3, RotaTeq #3, Flu #1
- 7 Months Flu #2
- 12 Months MMR #1 Var #1, PCV #4, Hep A #1
- 15 Months Dtap #4, Hib #4
- 24 Months Hep A #2
- 4-5 Years Dtap #5, IPV #4, MMR #2, Var #2, Hep A #1 (if not given)
- 11 - 13 Years Tdap, MCV #4, HPV#1, Hep A #1 (If not given)
HPV #2 (last dose) if received before 15 Year
HPV #3 (fast dose) If HPV #2 received after 15 Years - 16 Years MCV #2, Men & #1 (schedule Men B #2 in 1 Month)
